A current physical signed by a physician stating the athlete is in sound health. According to IHSA rules a current physical covers one calendar year. Thus, in most cases, a physical administered in the summer would cover all sports. Click here for IHSA Physical Form. Health Cards are handed out by individual coaches

An athlete must comply with all rules and regulations of the Illinois High School Association and Oswego School District 308.

Purchase of student school insurance unless waived by a parent or guardian. If school insurance is purchased, it should be noted that football insurance is a separate entity.

A student must not fail two subjects. If he/she does so on the weekly eligibility check, the athlete will be ineligible for the next week. If the athlete fails two half-credit (9 weeks) subjects in any term, the athlete becomes ineligible for the next term. If a student fails two full-credit (2 terms) classes, the student will be ineligible for the next two terms. Bottom line - pass everything. You are in school for an education.

A $60 user fee must be paid to the athletic office prior to the first scheduled contest of each sport. There will be a maximum charge of $120 per year for all extra-curricular activities. It shall be understood that payment of fees does not guarantee athletic participation. If an athlete drops or is removed from a program, such fees paid shall not be reimbursed
