Dear Parents/Guardians:
On Monday, April 14th some of our students will be
taking the Illinois Youth Survey sponsored by the
Illinois Department of Human Services. The purpose
of this survey is to gather information needed to
plan important prevention and intervention programs
to reduce youth drug use and associated risk
behaviors.
The survey is anonymous. Oswego East will receive a
report of the overall results, but the individual
student responses will not be known.
The survey is voluntary. Please contact the school
if you do not, or your child does not, want to
participate. You can contact Lisa Snow at

The survey will be given during Advisory.
A blank survey is available for review at the
administration office at Oswego East. Please contact
