Health Registration Information
Illinois law requires the following examinations:

Physical examinations are required for students entering pre-kindergarten, kindergarten, sixth grade, and ninth grade. The examination must have taken place on or after one-year prior to the first day of school.

Dental examinations are required for students entering kindergarten, second grade, and sixth grade. The examination must have taken place on or after November 15, 2012 for the 2013-14 school year.

Eye examinations are required for students entering kindergarten, or those just new to Illinois. The examination must have taken place on or after October 15, 2012 for the 2013-14 school year.
Parent Forum: Illegal Vices/Fatal Choices
More than 100 District 308 parents of teenagers, or soon to be teens, attended the district's second annual Parent Forum—Illegal Vices/Fatal Choices. The forum focused on issues of teen safety and parent accountability regarding the use of alcohol, synthetic drugs and illegal substances by minors.
