Health Registration Information

Illinois law requires the following examinations:

• Physical examinations are required for students entering pre-kindergarten, kindergarten, sixth grade, and ninth grade.  The examination must have taken place on or after August 26, 2008 for the 2009-10 school year. 
  • Certificate of Child Health Examination
  • Certificado de Examen de Salud del Nino
• Dental examinations are required for students entering kindergarten, second grade, and sixth grade.  The examination must have taken place on or after November 15, 2008 for the 2009-10 school year. 
  • Proof of School Dental Examination Form
  • Formulario Comprobante del Examen Dental Escolar
  • Dental Examination Waiver Form
  • Formulario de Renuncia Voluntaria del Examen Dental Escolar
• Eye examinations are required for students entering kindergarten, or those just new to Illinois.  The examination must have taken place on or after October 15, 2008 for the 2009-10 school year. 
  • Eye Examination Report
• An Annual Health History Form is required by the district for all District 308 students.  The information is shared with paramedics in cases of emergency.  It is important for health service personnel to be aware of children's medical needs.
