New Student Registration - Grades  K through 12

New to District 308? All new students to District 308 must register at the Central Registration Office. We will begin taking appointments for the 2009/2010 school year at the end of May.

Documentation Needed to Enroll a New Student:

  • ISBE 33-78 - Illinois State Board of Education Transfer Form – only required if transferring from a public school within the state of Illinois. No school district is required to admit a new student who is transferring from another Illinois school district unless he or she can produce the standard form from the student’s previous school district. Your previous school should provide you with this document.

  • Three proofs of residency are required:

    • Two from a current landline phone, electric, gas, water or cable TV bill (service address and mailing address must be listed); AND

    • Current real estate tax bill or Mortgage Statement, OR

    • Current apartment lease; OR

    • Closing papers – if purchased home is less than 12 months
      *Items NOT accepted: driver’s license, checkbooks, rent receipt, insurance papers, credit card statements, cell phone bills

  • Birth Certificate - County issued original (hospital copy not accepted).

  • Medical Information - current copy signed by an Illinois physician.

    • Physical: Required for all new students. If coming from out-of-state, you have 30 days to obtain a physical from Illinois.

    • Dental: Required for Kindergarten, 2nd and 6th grade.

    • Vision: New requirement for all students entering an Illinois school for the first time.

  • Dental - Students enrolling in Kindergarten, 2nd or 6th grade must have a dental examination dated after November 15, 2009.

  • Vision - New Requirement for any student entering an Illinois school for the first time.

  • Unofficial Transcripts for students entering grades 10, 11 or 12 or second semester 9th grade. Grades 6, 7 & 8 provide the final report card.

  • Current Schedule/Withdrawal Grades

  • Current IEP or any other Special Education records (if applicable)

  • Registration Forms - Registration forms are available here. Please complete them prior to attending registration.

  • Custody Papers (if applicable)

  • Tuition Student Pending Residency - An Application/Approval for Tuition Student form must be filled out and signed at the District Administration Center before a student can be registered. A construction contract, purchase contract or lease, signed by both the buyer and seller with a move in date must accompany the form.

Instructions for Completing New Student Registration Forms
Registration Forms requires minimum Adobe Acrobat Reader 6.0 to open and you must have a functioning printer connected to your computer. 

The registration packet may be completed in one of two ways

  1. Complete them on your computer and print them. Please note your information cannot be saved into the PDF document for later use. You must complete it in one sitting. The registration form should take less than 30 minutes to complete. It is recommended that you preview the forms first and have all the necessary information available before you begin.
  2. Print out the packet and fill in the various forms by hand.

Instructions for filling out the Registration Packet on your computer

  1. You will not be able to save the forms. Simply use your mouse to click on each blank line and enter the appropriate information. Items such as student name and dates will be automatically transferred from form to form. Please note that First Name, Middle Name, and Last Name need to be entered into their separate fields (click above First, Middle, and Last to select the individual field).
  2. It is recommended that you preview the forms first and have all the necessary information available before you begin; i.e., doctor's name and phone numbers, emergency contact address and phone numbers, and household email address.
  3. The physical, dental, and eye exam forms must be printed, completed by the appropriate health care provider and returned along with the registration packet when you register your children, or by the first day of school.
  4. Print the completed forms.
  5. Sign the appropriate pages.
  6. Bring your completed paperwork to registration.

Bring the following documentation to registration:

  1. County issued Birth Certificate
  2. Three (3) proofs of residency: current phone, electric, gas, water or cable bill, current tax bill, current apartment lease, current closing papers or mortgage.
  3. Current Physical
  4. Completed registration packet
  5. Payment to Oswego Community School District #308

Healthcare Forms
The physical, dental, and eye exam forms must be printed, completed by the appropriate health care provider and returned along with the registration packet when you register your children, or no later than the first day of school.
